Your initial appointment will consist of a consultation to determine your concerns, an examination to enable diagnosis and treatment planning, and if appropriate, dental prophylaxis (tooth cleaning). For patients in good oral health who have had regular professional dental care and follow-up, and who have no specific symptoms to investigate, dental prophylaxis may be scheduled at the first appointment. However, for many new patients, it is more appropriate to focus on discussing your concerns, and on the examination itself. When pain or specific symptoms are present, treatment may be accomplished on the initial visit. However, a complex medical history or treatment plan will require separate evaluation and treatment appointments.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable.
- A list of medications you are currently taking.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern during the initial examination or dental prophylaxis (tooth cleaning). Examples of these medical conditions include uncontrolled diabetes, high blood pressure, artificial heart valves, prosthetic joints, coronary artery stents, recent hospital or emergency room visits, or anti-coagulant medications. If you are not sure if a medical condition or medication is of concern, please mention it to our appointment coordinator, who will then discuss it with Dr. Shonberg.
If your dentist or physician has taken x-rays, you may request that they mail or e-mail them to our office. If there is not enough time, please pick them up and bring them to our office for your consultation. If additional films are necessary, they can be taken at our facility.
